DocuSign Envelope ID:53D55D40-1809-4F65-BAC5-CBC288F3AD4B <br /> NORTH CAROLINA <br /> CONTRACT AMENDMENT <br /> ORANGE COUNTY <br /> THIS CONTRACT AMENDMENT ("Amendment") is made and entered into this 8th day of November, 2021 by and <br /> between ORANGE COUNTY (hereinafter referred to as "County") and Catharine Rice (hereinafter referred to as <br /> "Provider"). <br /> WITNESSETH: <br /> THAT WHEREAS, the County and Provider entered into a contract dated 18 May 2021, (hereinafter the "Original <br /> Agreement"), for the provision of services for Broadband consulting, including strategic planning and advice, <br /> review and feedback on relevant documentation (e.g., RFP's, grant proposals, press releases), participation in <br /> Broadband Task Force meetings, other facilitation as needed. Also, additional insight into new ARPA <br /> (American Rescue Plan Act) funding rules, requirements and regulations." (See Attachment A) <br /> WHEREAS, the County and Provider desire to amend the Original Agreement while keeping in effect all terms and <br /> conditions of the Original Agreement not inconsistent with the terms and conditions set forth below. <br /> NOW THEREFORE, for and in consideration of the mutual covenants and agreements made herein, the parties agree to <br /> amend the Original Agreement as follows: <br /> 1. In order to ensure the completion of the Services identified in the term of the Original Agreement is amended <br /> to reflect an end date by which all Services shall be completed of 30 May 2022. <br /> 2. Attachment A to the Original Agreement is amended by adding the following tasks and services to the <br /> Services to be provided by the Consultant: Continued Broadband Consulting, including strategic planning <br /> and advice as well as continued participation in Broadband Task Force meetings. <br /> 3. Article 5, Section A is amended to reflect a maximum payable not-to-exceed amount of fifteen-thousand and <br /> no/100 dollars ($5,000 original contract, $5000 first amendment and additional $5,000.00 — 50 additional <br /> hours at$100/hour-for Amendment 2). <br /> 4. Except for the changes made herein, the Original Agreement shall remain in full force and effect to the extent <br /> it is not inconsistent with this Amendment.
