Please describe the impact the proposed programs will have on the target population. Please include <br /> specific quantitative and qualitative data in your response. <br /> In 2020, MOWOCNC increased meal distribution by 39%, distributing over 17,000 more <br /> meals, with a significant increase in Chapel Hill residents served. The award will help sustain <br /> this level of service. Delivery of healthy meals accompanied by friendly check-ins, help <br /> improve health, alleviate hunger and reduce isolation. Studies show that MOW recipients <br /> (average after 30, 90 & 120 days) have reduced emergency room visits (20%), hospitalization <br /> rates (36%) and nursing home usage (30%). Moreover, they report healthier eating, <br /> improvement in mental health, and reductions in the rate of falls, feeings of isolation and <br /> loneliness, and anxiety about aging at home. <br /> 2. What methods/tools will your organization use to evaluate the proposed program's effectiveness? <br /> Please include specific examples, such as a logic model. <br /> Inputs: Staff, Volunteers, Supplies, Equipment, Partnerships, Funding <br /> Outputs: Meals delivered, human connections, referrals. <br /> Outcomes: Maintain increased service to recipients. <br /> Reduced food insecurity. <br /> Reduced isolation/social isolation. <br /> Sustained ability to age in place. <br /> Evaluation:January 2021 new recipient survey will be launched that includes both food <br /> insecurity and social isolation and loneliness scales. This data will be compared against <br /> recipient intake questions as well as national Meals on Wheels data to evaluate the <br /> effectiveness of our program. Moreover, qualitative data collected from recipient interactions <br /> and conversations with staff or volunteers are captured, tracked and will be analyzed in <br /> conjunction with survey data. <br /> 3. Please briefly describe how your proposed programs aligns with evidence-based approaches to <br /> addressing human service need(s). <br /> The core of our service is the delivery of consistent, nutritious meals and friendly check-ins. <br /> Access to good nutrition is critical as older adults are often at risk of malnutrition given the <br /> biological, social, economic and functional challenges that often accompany aging and limit a <br /> senior's ability to acquire, prepare and consume nutritious foods. Research suggests that <br /> receiving MOW services reduces food insecurity rates by up to 28%.
