Insurance certificate issued to Owner by Contractor's insurance carrier listing required coverage <br />(if not already required for submittal as a part of execution of the construction contract.) <br />5. Written security plan, if required by the Owner. <br />6. Contractor Safety Program, specifically designed for this project that recognizes and mitigates <br />the specific hazards present in performing the Work. <br />7. Provide a list of any subcontractors to be utilized in performance of the work. Submit <br />information regarding the subcontractors including contact information, copies of <br />licenses/certifications, and references (if requested). <br /> <br />1.04 CONTRACTOR'S POST-JOB SUBMITTALS <br /> <br />A. Provide all original copies, unless otherwise noted, of each of the following post-job submittals: <br /> <br />1. Final Application for Payment (5 original copies) <br />2. Consent of Surety to Final Payment (if applicable) <br /> 2. Contractor’s Affidavit of Release of Liens (properly signed, notarized, etc.) <br /> 3. Properly executed release of liens by subcontractors and/or vendors <br /> 4. Certification letter that no asbestos containing materials were used. <br />5. Final list of all subcontractors and suppliers with names, addresses, and phone numbers <br />6. Specific operating and maintenance manual(s) for the new roof system and components. <br />7. Duplicate, notarized copies of the Contractor’s 5-year and two originals of the <br />Manufacturer’s warranty. Product and project warranties requiring Owner’s signature <br />are not acceptable. (Lengths of shingle warranties vary – see Section 073113 for <br />additional information) <br />8. One set of as-built drawings- including a copy of both design and shop drawings with <br />changes clearly marked (Red-Lined Drawings). <br /> <br />PART 2 – PRODUCTS – Not Used <br /> <br />PART 3 - EXECUTION <br /> <br />3.01 IDENTIFICATION OF SUBMITTALS <br /> <br />A. Number consecutively and clearly identify submittals. Submittal Form (SF-1) must accompany each <br />submittal package provided. This form will be provided electronically if requested. Show <br />identification on at least the first page of each submittal, and elsewhere as necessary for positive <br />identification of the submittal. <br /> <br />1. When material is resubmitted, cite the original submittal number for reference and add a <br />suffix such as "-A, -B" (2-A, 2-B, etc.). <br /> <br />B. Maintain an accurate submittal log for the duration of the Work, showing current status of all <br />submittals. Make the submittal log available to the Owner’s representative for their review. <br /> <br />C.
