<br />Orange County Outside Agency Performance Agreement Page 8 of 13 <br />Rev.9/21 <br />Exhibit A <br /> <br />Provider’s Revised Scope of Services and Program Budget <br /> <br />NC DHHS Grant Supporting Justice-Involved Individuals with Opioid and <br />Substance Use During COVID: Strategy 1A and 1B The purpose of the grant is to <br />provide a pre-arrest and post arrest diversion program, that diverts people who commit <br />low-level crimes to appropriate harm reduction, social/health services, treatment, and <br />recovery services in addition to a comprehensive re-entry planning and navigation <br />support program to support people in navigating treatment and suppor t options during <br />COVID-19, and connecting to the needed treatment, harm reduction programs, peer <br />support and other support services upon release. The CJRD is the recipient of this grant, <br />but Freedom House is a critical sub-contractor. As the attached Budget Narrative <br />indicates, the Freedom House will dedicate 2.5 FTE positions to supporting the grant and <br />will provide treatment, peer support, harm reduction and medication assisted a treatment <br />management. <br /> <br />SCOPE OF WORK (PERFORMANCE REQUIREMENTS) <br /> <br />Required elements of services for Strategy 1A: <br /> Expand and enhance outreach for the current Orange County Coordinated Opioid Overdose <br />Reduction Effort (COORE). <br /> Expand Orange County Pre-Arrest Diversion Program (OC-PAD) and hire a Recovery Diversion <br />Coordinator to assist in critical pre-arrest and post-arrest diversion from the justice system, harm <br />reduction outreach and linkage to treatment and services. <br /> Collaborate with law enforcement and the District Attorney’s Office to ensure they use their <br />discretion to divert individuals with substance use disorders who commit low-level misdemeanor <br />offenses from the criminal justice system and provide these individuals with accountability, referral <br />to services and provide information about the collateral consequences of criminal justice <br />involvement. <br /> Collaborate with Freedom House Recovery Center as a sub-contractor to employ a Peer Support <br />Specialist, Clinician and Nurse Practitioner to provide Moral Reconation Therapy, Peer Support, <br />and MAT. <br />Required elements of services for Strategy 1B: <br /> Screen those incarcerated in the Orange County Detention Center and those recently released <br />from state prison for SUD with priority placed on serving those with Opioid Use Disorder and those <br />who are members of marginalized communities. <br /> Engage incarcerated individuals screened for SUD in evidence-based treatment prior to release <br />and/or immediately following release from incarceration to promote recovery. <br /> Provide harm reduction education and Naloxone kits immediately upon release to help reduce t he <br />substantial risk of overdose <br /> Hire a dedicated Reentry Recovery Navigator with lived experience to work in the jail and <br />community to connect consumers to behavioral health treatment and other needed services, <br />including COVID-19 education and testing. <br /> Collaborate with Freedom House Recovery Center as a sub-contractor to employ a Peer Support <br />Specialist, Clinician and Nurse Practitioner to provide Moral Reconation Therapy, Peer Support, <br />screening and engagement at the detention center and MAT, Peer Support and MRT post-release, <br />to ensure early engagement while incarcerated and a seamless transition back to the community. <br /> <br /> <br /> <br /> <br />DocuSign Envelope ID: C2326517-F841-457F-8D35-A1184260BE2B
