Exhibit A <br /> <br />Scope of Work <br />Purpose: Chatham County Cooperative Extension aims to increase blood pressure monitoring and fruit <br />and vegetable consumption among individuals with hypertension through education and practical <br />instruction. The curriculum will include nutrition education for management of hypertension, food <br />safety and basic cooking skills for preparing fruits, vegetables, and other nutritious food options. The <br />series will also include a focus on local food access with the opportunity to purchase from local farmers <br />and producers made available through vouchers. <br /> <br />Project Scope: <br />Chatham County Cooperative Extension Duties <br /> Enroll participants into educational series. Provide evidenced-based nutrition education for <br />managing hypertension and food safety and preparation skills. <br /> Connect participants with local farmers and producers to purchase foods discussed in <br />educational series. <br /> Assist participants in monitoring blood pressure throughout the series and discuss the <br />importance of monitoring. <br /> Provide relevant incentive items so participants may practice skills they’ve learned as home. <br /> Provide and manage a voucher system that allows participants to purchase nutritious foods <br />from local farmers and producers. <br /> Evaluate change in frequency of monitoring blood pressure and consumption of fruits and <br />vegetables with pre and post-tests. <br /> Provide reports on classes, participation, and evaluation outcomes to Orange County Health <br />Department. <br /> <br />OCHD Duties <br /> Assist Chatham County Cooperative Extension by providing the following funding: <br />o Farmer’s Market Vouchers of $10 each per participant (up to 20 participants) for 10 <br />weeks totaling $2,000. <br />o Farmer’s Market Program teaching tools (thermometers, cutting board, containers) <br />totaling $1,000. <br />DocuSign Envelope ID: 5710DA8F-5574-4B1B-9399-537F26BE53DA
