(“Meridian”) provides services to Client which is currently <br />using One-X Attendant R4 and will be upgrading their Avaya Aura Solution to R8. <br /> <br />CHANGE TO SCOPE <br />This Change Order (“CO”) documents the requested services, associated responsibilities, assumptions and corresponding <br />charges. Once approved, this CO authorizes Meridian to perform the following out of scope services: <br /> <br />Voicemail Message Migration <br />All Passwords, Greetings and Messages will be migrated from a CM Messaging R7 Platform to an already built Avaya <br />Messaging System for nine hundred ten (910) users. <br />Meridian will provide support for any configuration requirements needed for the voicemail platform message migration <br />tasks. <br /> <br />CLIENT CONTACTS <br />The client contact listed above has been identified for directing the specific activities of their respective employees and have <br />sufficient authority to represent Client, on matters arising in connection with the performance of the respective services. Client <br />may replace the identified persons on written notice to the other party. <br /> <br />CLIENT RESPONSIBILITIES <br />This CO is subject to the same responsibilities identified in the main agreement. <br /> <br />Additional CO responsibilities: <br />Manage the collection of customer data using provided data collection forms. <br />Coordinate and ensure compliance with provided technical requirements. <br />Provide information and guidance on Client Change Management policies. <br />Assist the assigned PM as necessary with scheduling and coordination of Client resources. <br />Provide any requested documentation or information necessary to adhere to the project schedule. <br />Coordinate and provide any necessary access to Client systems. <br />Attend all mutually agreed upon required project meetings (if applicable). <br />Coordinate all Client provided vendors, subcontractors, and LEC/Carrier interactions. <br />Provide proper site environment as defined in the product documentation. <br />Provide floor plans and cabling schematics as applicable. <br />Ensure that the installation of network and dial circuits is complete. <br />Ensure that demarcation points can be connected to with the cables provided. <br />Provide access to all installation locations and equipment being installed. <br />Provide an on-site contact to assist during installation. <br />Provide all IP addressing for customer’s network. <br />Provide Layer 1, 2 and 3 IP infrastructures all network connectivity unless provided under this agreement. <br />Meridian IT Inc. - CONFIDENTIAL <br />Proposal #: 013620 v1 <br />OppQ #: 145568 <br />3/5 <br />320 N.
