Scope of Work (Rev. 06/11) <br />SCOPE OF WORK <br /> BACKGROUND <br />These Emergency Solutions Grant-Corona Virus (ESG-CV) funds must be used to prevent, prepare <br />and respond for and respond to coronavirus among individuals and families who are homeless or <br />receiving homeless assistance and to support additional homeless assistance and homeless <br />prevention activities to mitigate the impacts created by the coronavirus. Expenses incurred beginning <br />March 10, 2020 directly related to coronavirus are eligible for reimbursement. <br /> PURPOSE <br />The purpose of this contact is: <br />ESG funds are intended to be used as part of a crisis response system using a low <br />barrier, housing-focused approach to ensure that homelessness is “rare, brief, and one <br />time”; <br />Engage homeless individuals and families living on the street; <br />Improve the number and quality of emergency shelters for homeless individuals and <br />families; <br />Help operate emergency shelters; <br />Provide essential services to emergency shelter residents; <br />Rapidly re-house homeless individuals and families; <br />Prevent families and individuals from becoming homeless. <br />Services to be provided: <br />Street Outreach - This is a HUD defined service that funds the ability of the sub <br />recipient <br />to perform outreach services to individuals that are unsheltered and living on the street <br />on in locations unfit for human habitation. <br />RRH Services: Rapid Re-housing is a service provided to rapidly re-house individuals <br />who are homeless or at risk of becoming homeless. Funds can be used both for <br />financial services and case management. <br />• <br />PERFORMANCE REQUIREMENTS <br />Reimbursement submission must be accurate to ensure both timely service provision and <br />reimbursement. Incorrect submissions delay and effect spending. All requisitions incorrect or <br />incomplete will be returned for correction and will not be submitted for payment until corrections are <br />received. Requisitions must be submitted monthly and within 45 days of the expenditure disbursing <br />ESG-CV funds per HUD regulation. Failure to stay if program compliance and/or to develop a <br />corrective action plan as a result of findings deriving from a monitoring review may result in loss of <br />funding. <br /> PERFORMANCE STANDARDS <br />Monitoring by the ESG office will yield 10% or less in the findings/categories in cumulative records <br />reviewed.
