2 <br />NC ESG CV Scope of Work <br /> <br /> <br /> <br /> <br />ESG-CV CAPER Submission::Required quarterly by HUD; dates to be determined. <br /> <br /> <br />PROGRAM COMPONENTS and PROGRAM ELIGIBILITY <br /> <br />Agencies participating in the NC ESG program and/or receiving NC ESG funds, are eligible to perform activities under the areas <br />provided below. Each component must serve eligible participants as defined under 25 CFR 576.2 and the NC ESG 2020 Desk <br />Guide. <br /> <br />Street Outreach <br />Meet the immediate needs of unsheltered homeless people by connecting them with emergency shelter, housing, <br />and/or critical health services. <br /> <br />Eligible Activities: <br />• Contact Activities <br />• Engagement Case Management <br />• Emergency Health Services <br />• Emergency Mental Health services <br />• Transportation Costs <br />• Services for special populations (homeless youth, homeless persons with HIV/AIDS, etc.) <br /> <br />Emergency Shelter <br />Intended to increase the quantity and quality of temporary emergency shelters provided to homeless people by <br />supporting the shelters operating expenses and essential services. <br /> <br />Shelter Operations <br />• Minor or routine maintenance <br />• Rent <br />• Security <br />• Fuel <br />• Insurance <br />• Utilities <br />• Furnishings (under $500 per item; see desk guide) <br />• Equipment (under $500 per item; see desk guide) <br />• Hotel or motel vouchers when no appropriate emergency shelter is available <br /> <br />Essential Services <br />• Case management <br />• Childcare, education, employment, and life skills services <br />• Legal services <br />• Mental health <br />• Substance abuse services <br />• Transportation <br />• Services for special populations <br /> <br />Rapid Rehousing <br />Move homeless individuals and families quickly into permanent housing through rental assistance , housing relocation <br />and stabilization services.
