<br />Page 1 of 2 <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br /> <br />PLM Equipment Services Inc. <br />212 Powell Dr, Suite 122 <br />Raleigh, NC 27606 <br />919-233-2231 <br />Fax 919-233-3480 <br />www.emsarncva.com <br /> <br /> ADDENDUM # 1 <br />The Gold Plan <br />“All Inclusive Agreement” <br />The “All Inclusive Plan” incorporates all of the foregoing standard preventive program plus all <br />labor and parts for one guaranteed price for two, three or four visits per year. <br /> <br />The program includes, but is not limited to the following: <br /> <br /> <br /> Worn Wheel Bearing <br /> Side Arm Pins with Knobs and Springs <br /> Head End Release Handles <br /> Foot End Release Handles <br /> Lift Handles <br /> Crutch Tips <br /> Scuff Strips <br /> Leg Return Springs <br /> <br />The Gold level also includes the five point equipment management program, consisting of: <br /> Preventive Maintenance <br />Cleaning <br />Inspection <br />Lubrication <br /> Equipment Tune-Up <br />Hardware Tightening <br />Alignment Adjustments <br /> Equipment Evaluation <br />Reveals Needed Repairs <br />Detects Potential Problems <br /> Usage Evaluation and Training <br />Reveals Improper Usage <br /> EMSAR Equipment Report <br />Provides a Documented History of Preventive Maintenance <br /> <br />This service level includes the five-point maintenance program and all parts and repairs for a <br />price which will remain fixed for the duration of the service contract. The inclusive agreement also <br />provides, at no charge, for one emergency return trip. Any other emergency return trip will be <br />billed for travel, labor and parts. Abuse is not covered as described by the manufacturer. <br /> <br /> Optional Accessories <br /> <br />Repairs will be made if possible (parts may not be available), but devices can not be <br />replaced under this agreement.
