Have you <br />experienced a <br />decrease in TOTAL <br />HOUSEHOLD <br />income of 30% or <br />more? <br />Yes <br />Please explain how <br />your general <br />livelihood has been <br />impacted by COVID- <br />19. <br />ALL of my performances hav e been canceled since the middle of <br />March. Some were rescheduled for the Fall but they hav e already <br />been canceled now. The bulk of my income is from performance. I <br />also teach priv ately and after sev eral weeks of no teaching I was able <br />to start online lessons for a reduced rate and less than half of my <br />usual teaching load. I would normally be adding new students <br />throughout the year but that has not happened at this time. <br />The total amount of income lost due to prev iously scheduled <br />performances is ov er $5800 for the Spring concert season alone. <br />Normally there would be more performances added to my schedule <br />each week. The canceled performances were all scheduled in late <br />2019 and early 2020. No new performances hav e been scheduled in <br />2020 at this time. <br />Have you applied for <br />unemployment <br />benefits? <br />No <br />Please explain your <br />decision to not apply <br />for benefits: <br />Started the process but the system was not accepting applications <br />from self-employed indiv iduals. Tried to get help but failed and grew <br />frustrated with the process and gav e up. <br />Grant amount <br />requested: <br />600 <br />How do you intend to <br />use the grant, if <br />awarded? <br />At this point just trying to pay bills. Nothing noble, sorry. Sav ings ran <br />out.
