Exhibit “A” <br /> <br />Scope of Services – FY2020-2021 <br />Outside Agency Performance Agreement <br /> <br />Agency Name: El Futuro, Inc. <br />Program Name: Avanza: Mental Health Access & Engagement for Latino Immigrant Families <br />Funding Award: $54,969 <br /> <br />Outline how the agency will spend Orange County’s funding award <br />Expense Description Amount <br />Personnel salaries $54,969 <br /> <br /> <br />Program Services <br />Outline the critical services (activities) the agency will employ to attain the Anticipated Outcomes below, by <br />June 30, 2021. <br />• Provide direct mental health and substance use treatment services to Orange Co Latino residents (via <br />telehealth during COVID) <br />• Provide school-based mental health treatment to Orange Co Latino students (via telehealth during <br />COVID) <br />• Provide parent psycho-education skill-building groups, parent consultations, and support/training for <br />teachers to support the emotional / behavioral health of Orange Co’s Latino students (actively planning <br />for COVID adaptations) <br />• Provide all services via telehealth during COVID <br /> <br />Anticipated Outcomes <br />The Anticipated Results column must include quantifiable results in the form of number of persons / units <br />served within Orange County, only (all Towns and municipalities).
