The total amount funded under this Amendment is $4,025,269. The Division of Public Health, <br />Office of Public Health Preparednes:c and Response has made (these) funds available to Local Health <br />Departments (LHD) through a fundnig formula that awards Local County Health Departments and Multi <br />County Districts with specific budgeted amounts based on population percentages (60%j, percentage of <br />land square miles (20%) and percentage of total square miles (20%). Additionally, a base amount of <br />$50,000 will be received (awarded) ley each county or district. <br />These funds are made available to L~~cal (County) Health Departments and Multi County Districts to <br />further assist counties in continuing to strengthen local public health infrastructure and capacity, and to <br />effectively respond to a possible bio':errorism event. Specifically, under our Continuation Guidance from <br />the Centers for Disease Control and Prevention (CDC), these funds will be used to enhance strategic <br />direction, coordination and assessment, public surveillance and detection capacities, risk communication <br />and health infomnation disseminatio:z, and education and training. <br />Scope of Work for Local Health Deuartments: <br />• Establish a process for strategic leadership, direction, coordination, and assessment of activities to <br />ensure local readiness, interagency collaboration, and preparedness for bioterrorism, other outbreaks of <br />infectious disease, and other public health threats and emergencies. <br />• Conduct integrated assessments of public health system capacities related to bioterrorism, other <br />infectious disease outbreaks, and other public health threats and emergencies to aid and improve <br />planning, coordination, and imf~lementation. <br />• Respond to emergencies caused. by bioterrorism, other infectious disease outbreaks, and other public <br />health threats and emergencies ~:hrough the development, exercise, and evaluation of a comprehensive <br />public health emergency preparedness and response plan. Exercise this plan at least annually and <br />incorporate any lessons learned from exercises into successive versions of the plan. <br />Effectively manage elements oi'the CDC Strategic National Stockpile (SNS) at the county or local <br />level, should it be deployed-translating SNS plans into fum preparations, periodic testing of SNS <br />preparedness, and periodic training for entities and individuals that are part of SNS preparedness. At a <br />minimum, planning and exercises should incorporate the reception of SNS materials at the county's <br />Local Disinbution Site, distribi:tion to hospitals and Dispensing Sites within the county, and conduct <br />of dispensing pharmaceuticals toaffected personnel. <br />Page _ of _ ~` , <br />County Health )~' ecto Signature and Date <br />
