Provide vouchers that enable “Qualified Residents” (as defined in this <br />Agreement) of Orange County to spay or neuter a dog or cat. For each <br />spay/neuter surgery performed pursuant to this Agreement, Provider shall <br />receive and review all submitted applications and issue vouchers to Qualified <br />Residents, including those residents qualified by DSS. Provider shall be <br />responsible for issuing vouchers in a timely manner to DSS Qualified <br />Residents. <br /> <br />iii. Partner with a licensed veterinarian to deliver spay/neuter surgeries. Provider <br />shall have the sole responsibility for locating, contracting and otherwise <br />providing spay/neuter options through veterinarians or veterinarian clinics. <br />The County shall not be required to enter into any agreements with <br />veterinarians or veterinarian clinics for the provision of spay/neuter <br />contemplated by this Agreement, <br /> <br />b. Orange County will qualify residents for financial assistance in the low income <br />spay/neuter program in the following: <br /> <br />i. DSS will determine, in its exclusive discretion, which Qualified Residents meet <br />the criteria for full coverage surgeries in which no co-pay is required for <br />spay/neuter surgery. DSS shall coordinate with the County Animal Services <br />Department (“Animal Services”) to identify and submit applications to <br />Provider for these Qualified Residents. <br /> <br />ii. Animal Services will have sole responsibility for coordinating with the <br />Department of Social Services to make information known to their clients <br />about the “no cost sterilization” for dogs, cats and other small animals, and <br />for providing, transmitting, and otherwise facilitating the submission of <br />applications to Provider for Qualified Residents as determined by DSS. <br /> <br />c. The Scope of Services may be different from the original application based on County <br />appropriation; however, any revisions or amendments to this Agreement must be <br />approved in writing by the County and attached to this Agreement. <br /> <br />d. The Provider shall be solely responsible for the means, methods, techniques, sequence, <br />safety program and procedures necessary to properly and fully complete the work set <br />forth in the Scope of Services. <br /> <br />4. Funding. <br /> <br />a. The County agrees to appropriate for the provision of services described in Scope of <br />Services up to a maximum sum of Twenty Thousand Dollars ($20,000). <br /> <br />b. The Provider will use funds paid pursuant to this Agreement to supplement Provider’s <br />existing AnimalKind Program. All funds payable to Provider pursuant to this <br />Agreement must be used for the benefit of applicants residing in Orange County.
