Box 8181 <br />Hillsborough, NC 27278 <br /> <br />The Chapel Hill Training Outreach Project-KidSCope and the Orange County Health <br />Department agree to work collaboratively in the fulfillment of the Assuring Better Child Health <br />and Development (ABCD) project. <br />The ABCD model is a proven, universal approach to screening young children in primary health <br />care settings. It aims to increase the rates of developmental screening and referral rates for young <br />children within the medical home by integrating routine developmental screening into well-child <br />visits using a validated tool such as the Ages and Stages Questionnaire (ASQ) or the Parents <br />Evaluation of Developmental Skills (PEDS). <br />Both parties of this agreement are fully committed to working closely with each party to <br />fulfillment the ABCD project in our region. <br />The Orange County Health Department agrees to the following: <br />- Provide population data quarterly as needed. <br />- Participate in chart reviews every 6 months. <br />- Participate in quality improvement and technical assistance activities with ABCD <br />Coordinator. <br />The Assuring Better Child Health & Development Coordinator agrees to the following: <br />- Work closely with appropriate CCNC staff to provide ABCD services in medical <br />practices with a coordinated approach. <br />- Conduct chart reviews every 6 months and provide clinics reports and feedback reflecting <br />the data obtained. <br />DocuSign Envelope ID: 09C710EC-3DFB-4AC3-977B-18DF4D035521
