2 <br /> <br />g. completing a criminal background check on each student. <br /> <br />h. requiring students to comply with any requirements of Agency regarding <br />immunizations. Any immunizations required which student does not yet have must be <br />obtained prior to beginning student’s experience and the cost for the immunization <br />shall be paid for by student or student’s own medical coverage. <br /> <br /> <br /> 2. To terminate an individual student's experience if the learning activity is shown to be <br />detrimental to client welfare. <br /> <br /> 3. To require students to carry individual health insurance. <br /> <br /> 4. To inform students and faculty that they must be responsible for their own transportation <br />including parking while involved in the learning experiences associated with the Agency. <br /> <br /> 5. To the extent allowed under the North Carolina Tort Claims Act, The University of North <br />Carolina at Wilmington for its Public Health Program shall indemnify, hold harmless and <br />defend Agency, its officers, directors, employees and agents from and against any and all <br />losses, liabilities, claims, damages, causes of action, costs and expenses, including <br />reasonable attorney’s fees and litigation costs arising from University’s breach of any <br />covenants or obligations set forth in this Agreement. This obligation shall survive the <br />termination of this Agreement. <br /> <br /> <br />B. Orange County Health Department agrees: <br /> <br /> 1. To designate an appropriate Agency representative to be responsible for maintaining <br />liaison with the Public Health Program. In this connection the responsibility of this <br />person will be: <br /> <br />a. To supply information concerning the Agency, staff, and clients that will facilitate the <br />students' learning experience(s). It is understood that protected health information <br />will be kept confidential. <br />b. To provide online HIPAA training and OSHA Bloodborne Pathogen training prior to <br />their field experience. <br /> c. to communicate periodically with the faculty to: <br /> <br /> 1) discuss the nature of the students' experiences; <br /> <br /> 2) Monitor the quality of services provided by the student. <br /> <br /> d. to provide an adequate number of learning opportunities for student experiences; <br /> <br />DocuSign Envelope ID: 70A573EC-FF20-4FBA-81B1-B0CAEEC9FD7C
