<br />2.6 PROPOSAL CONTENTS- Revised <br />Vendors shall populate all attachments of this RFP that require the Vendor to <br />provide information. Vendor RFP responses shall include the following items and <br />those attachments should be arranged in the following order: <br />a) Cover letter: Identify Project Manager - name and email <br />b) Title Page: Include the company name and Federal ID number or Social Security <br />number <br />c) Vendor information: <br />o Company/Firm name <br />o Physical address and mailing address <br />o Email address & name of primary contact at each company/firm <br />o Main telephone number and direct telephone number for contacts <br />o Organizational Structure: a brief description of the ownership structure of <br />the Organization. Vendors must identify what type of business it is, where it <br />is organized (what state) and where the principal office is located. <br />o Number of years in business for each company/firm. <br /> <br />d) Regarding litigation with owners, subcontractors, and other construction -related <br />entities, list any active or pending litigation and explain. List, and briefly describe any <br />and all legal actions for the past three years in which respondent has been a debtor <br />in bankruptcy, a defendant in a lawsuit for deficient performance under a contract or <br />agreement; a respondent in an administrative action for deficient performance, or a <br />defendant in a criminal action. Failure to fully comply with is item will be grounds <br />for elimination from the competition. <br />e) Has the firm ever failed to complete any work awarded to it or has it been removed <br />from any project awarded to the vendor? Explain. <br />f) A written narrative response to address: <br /> Project Organization-Original three (3) addresses and Campus Wide Fiber Project <br /> Project Organization-Alternate A Project - optional <br /> <br />g) A written narrative response to address Project Timeline and Methodology <br /> Project Organization-Original three (3) addresses and Campus Wide Fiber Project <br /> Project Organization-Alternate A Project- optional <br /> <br />h) Vendor Experience: Give three (3) references to which your team has provided <br />services of a nature and quality similar to those required herein.
