Program Goal #5 <br />Program information <br />NORfll CAROliNA <br />Outside Agencies/Human Services <br />90% or 110 of Club Nova members will not experience <br />re-hospitalization due to psychiatric concerns. <br />100% or 117 of Club Nova members will report having <br />access to health care with 50% or 59 members receiving <br />primary care. <br />Each person who needs crisis prevention and <br />intervention services will receive those supports from <br />Club Nova. <br />All members needing assistance with securing and <br />maintaining benefits will receive that assistance. <br />85% (100) will participate in nutritious and affordable <br />meals (See Next Goal) <br />Outreach will be conducted through cards, calls, and or <br />visits to all members -100% (117). <br />100 % ( 117) will participate in the work-ordered day that <br />provides a sense of community, common purpose, and <br />opportunities to contribute building confidence, skills, <br />and self-worth. <br />17 % (20) will participate in educational endeavors. <br />85% (100) will participate in our Social <br />(Evening!Weekend/Holiday)Programming <br />Club Nova will address food insecurity and diet related <br />disease by providing at least 12,000 healthy, affordable <br />meals, engaging at least 80% or 93 clubhouse <br />members in meals, and providing education and training <br />related to cooking, nutrition, obtaining and growing food. <br />Club Nova partners with local food organizations to <br />secure approximately 750 pounds of fresh, local <br />roduce and meat Members and staff cook <br />Page 25 of 35 <br />EXHIBIT A: PROVIDER'S OUTSIDE AGENCY APPLICATION <br />DocuSign Envelope ID: D48DD7FB-D87E-4B89-8F49-216C48CF34E2
