PLEASE READ IT CAREFULLY. <br /> <br /> DISCIPLINARY ACTION COVERAGE <br /> <br />This endorsement amends the insurance provided under the following: <br /> <br />SOCIAL SERVICE PROFESSIONAL LIABILITY COVERAGE FORM <br /> <br /> <br />We will reimburse an Insured for reasonable attorney’s fees, costs and expenses incurred in responding to a <br />“disciplinary action” by reason of an act, error or omission of that insured in the performance of a “professional service” <br />committed during the time that the policy to which this endorsement is attached was in effect. The “disciplinary action” <br />must be reported to us prior to the time that the attorney’s fees, costs and/or expenses are incurred and we must <br />provide our written consent prior to the incurring of such fees, costs and/or expenses. The maximum amount payable <br />under this Additional Coverage, regardless of the number of “disciplinary action(s)” or the number of Insureds <br />responding to a “disciplinary action”, shall be $10,000 per policy period. We have the right but not the obligation to <br />defend any “disciplinary action” or to pay any fine, or penalty award resulting from any “disciplinary action”. <br /> <br />“Disciplinary action” is defined to mean an action or proceeding brought against an Insured by or in front of any <br />regulatory agency, disciplinary board, or peer review committee alleging misconduct in the providing of a “professional <br />service”. “Disciplinary action” does not include a criminal action or proceeding. <br /> <br /> <br /> <br /> <br />DocuSign Envelope ID: 81A61471-345A-47F9-8A2D-36F7929A6637
