PLEASE READ IT CAREFULLY. <br /> <br />DESIGNATED PROFESSIONAL SERVICES EXCLUSION <br /> <br />This endorsement modifies insurance provided under the following: <br />COMMERCIAL GENERAL LIABILITY COVERAGE FORM <br /> <br /> <br />SCHEDULE <br /> <br /> <br />Any and all “professional services” except to the extent that coverage is provided by the Social Service <br />Professional Liability Coverage Form. <br /> <br /> <br /> <br /> <br />With respect to any “professional services” shown in the Schedule, this insurance does not apply to “bodily injury”, <br />“property damage”, or “personal and advertising injury” arising out of an act, error or omission committed in the <br />performance of “professional services” by you or by any other person for whose act, error or omission you are legally <br />responsible. <br /> <br />“Professional services” is defined to mean conduct arising out of a vocation, calling, occupation or employment <br />involving specialized knowledge, labor or skill, and the labor or skill involved is predominantly mental or intellectual, <br />rather than physical or manual; including but not limited to acts, errors or omissions committed by individuals in their <br />capacities as an acupuncturist, adoption services employee, aide, assisted-living provider, childcare worker, <br />chiropractor, CNA, counselor, daycare provider, educator, home health aide, instructor, LPN, mentor, nurse <br />assistant, nutritionist, optician, phlebotomist, psychiatrist, psychologist, RN, resident home care provider and <br />supervisors, social worker, teacher, therapist, tutor or veterinarian. <br /> <br /> <br /> <br />DocuSign Envelope ID: 81A61471-345A-47F9-8A2D-36F7929A6637
