<br /> <br />ANI-RRG-NPO-001 05 20 <br /> <br />Page 8 of 8 <br /> <br />“Outside aggressor event” means an attack at or adjacent to your business premises by a person or <br />group or persons, not your employee or employees, actively engaged in killing, attempting to kill or <br />causing serious physical injury to a person or group of persons. <br />“Protected health information” means any information, whether oral or recorded in any form or medium: <br />1. That relates to the past, present or future physical or mental condition of an individual; the <br />provision of health care to an individual; or the past, present or future payment for the provision <br />of health care to an individual; <br />2. That identifies the individual or with respect to which there is a reasonable basis to believe the <br />information can be used to identify that individual; and <br />3. as defined within the Health Insurance Portability and Accountability Act of 1996, 42 U.S.C. <br />§1320d-1320d-8 (“HIPAA”) and other similar federal, state or local laws or statutes. <br /> <br />“Relative” means the spouse, parent, child (including a ward or foster child) or legally recognized <br />domestic partner of a “Member”. <br />"Reward" means the reasonable amount that you pay a person for information regarding a "security <br />event" or threatened or actual extortion covered by this policy; provided that the information is not <br />otherwise available and leads to the arrest and conviction of a person responsible for the “security event” <br />or extortion. <br />"Security event" means: <br /> 1. An act, error or omission that results, during the policy period, in unauthorized access or <br />unauthorized use of your computer system; or <br /> 2. Unauthorized or unexpected interference by anyone that restricts or prevents access, during the <br />policy period, to a computer system by persons who are authorized to gain such access; or <br />3. Infection of your computer system, during the policy period, by corrupting or harmful computer <br />code. <br />"Unsatisfied contribution" means a contribution of money, goods, services or property, pledged to you in <br />writing, that is not honored because of the donor's bankruptcy, reorganization, unemployment or <br />incapacitation where such bankruptcy, reorganization, unemployment or incapacitation prevents the <br />donor from fulfilling its terms of the contribution. <br /> <br />“Workplace violence incident” means any intentional threat or act of deadly force, including stalking <br />and/or actual or attempted suicide, occurring on your premises and resulting in “bodily injury” to your <br />employee(s), to your guest(s), or to your business invitee(s). <br /> <br />DocuSign Envelope ID: 81A61471-345A-47F9-8A2D-36F7929A6637
