The submittal should include the names, office locations, professional <br /> license numbers (if applicable) and general roles of the project team <br /> members, inclusive of any proposed sub-contractors. <br /> c. A description of the current workload for identified individuals. <br /> 5. A minimum of three references for similar projects (Maximum of 10 pages of <br /> content (i.e. 10 single-sided pages or 5 front and back pages) including: <br /> a. The name and date of the project, <br /> b. The location of the client, and <br /> c. A contact name including relevant contact information. <br /> d. At least one of these references should be able to describe the proposed <br /> project manager's experience and qualifications. <br /> 6. Evidence of lead Provider's professional liability insurance <br /> ($1,000,000/$2,000,000) and the ability of the lead Provider to meet the "Standard <br /> Risk Profile" of the Orange County Minimum Insurance Coverage Requirements. <br /> Lead Providers must cite in their response any and all claims that have been filed <br /> against the professional liability insurance carried by the lead Provider within the <br /> last five (5) years. Prior to contract execution, lead Provider shall obtain, at its sole <br /> expense, Professional Liability Insurance, Commercial General Liability Insurance, <br /> Automobile Insurance, and Workers' Compensation Insurance as described in the <br /> Orange County Risk Transfer Policy and Orange County Minimum Insurance <br /> Coverage Requirements (each document is incorporated herein by reference and <br /> available at: <br /> http://www.orangecountync.gov/departments/purchasing division/contracts.php). <br /> 7. E-Verify: HB789 imposes E-Verify requirements on contractors who enter into <br /> certain contracts with state agencies and local governments. The legislation <br /> specifically prohibits governmental units from entering into certain contracts <br /> "unless the contractor and the contractor's subcontractors comply with the <br /> requirements of Article 2 of Chapter 65 of the General Statues." (Article 2 of <br /> Chapter 65 establishes North Carolina's E-Verify requirements for private <br /> employers.) It is important to note that the verification requirement applies to <br /> subcontractors as well as contractors. The new laws specifically prohibit <br /> governmental units from entering into contracts with contractors who have not (or <br /> their subcontractors have not) complied with E-Verify.
