The contractor <br /> shall adhere to all of the performance requirements/standards in the scope of work, including <br /> performance measures in the performance measures chart below. <br /> Measure Output Reporting Monthly <br /> Type Frequency <br /> Measure All subrecipients must submit 1 requisition per month for reimbursement or must <br /> submit requisition documentation that outlines the activities and amounts spent <br /> toward their advance <br /> Budget Year 1 Trend Maintain <br /> Baseline amount of funds awarded or advanced <br /> Value <br /> Target Value 100% of the allocation by the end of the contract period <br /> Data Source fiscal data monitoring and requests for reimbursement <br /> Collection reimbursement submission <br /> Process and <br /> Calculation <br /> Collection monthly <br /> Frequency <br /> Measure Outcome Reporting Monthly <br /> Type Frequency <br /> Measure Subrecipient will use CRF award to meet CARES Act and Department goals, as <br /> described and approved in Subrecipient CRF request, to serve individuals <br /> experiencing homelessness stay safe from COVID-19 within CRF timeframe. <br /> Budget Year 1 Trend Maintain <br /> Baseline eligibility verification <br /> Value <br /> Target Value 100% <br /> Data Source electronic submission and file submission <br /> Performance Measures (Rev. 4//12) <br />
