DocuSign Envelope ID: BOFE06B7-7OC7-44D2-A46C-D41E904EED9B <br /> Contract Number 00041659/ Page 9 of 22 <br /> SCOPE OF WORK <br /> PURPOSE <br /> These Coronavirus Relief Funds (CRF) must be used on necessary expenses incurred due <br /> to the public health emergency with respect to the Coronavirus Disease 2019 (COVID-19). <br /> Expenses incurred beginning March 1, 2020 directly related to coronavirus are eligible for <br /> reimbursement. <br /> Street Outreach -$5,000 <br /> Rapid Rehousing -$49,337 <br /> Other- $140,663 <br /> Funds may be provided through advancements or reimbursement. If funds are received <br /> through advancement,fifty percent (50%) of the total award may be advanced upon contract <br /> execution. The remaining 50% may be advanced upon expenditure of 80% of the first <br /> advance. Any unspent advanced funds must be returned to the Department of Health and <br /> Human Services, Division of Aging and Adult Services by January 30, 2021. <br /> Requisitions for reimbursement or reports of expenditure of the advancement must be <br /> submitted at least monthly. <br /> Scope of Work(Rev. 06111) <br />
