DocuSign Envelope ID:440BABFB-21C5-40A5-B4F8-7B5096EFBBE9 <br /> MEMORANDUM OF AGREEMENT BETWEEN <br /> THE UNIVERSITY OF NORTH CAROLINA AT CHAPEL HILL <br /> FOR ITS SCHOOL OF PUBLIC HEALTH AND <br /> ORANGE COUNTY HEALTH DEPARTMENT <br /> WHEREAS, the University of North Carolina at Chapel Hill for its School of Public Health is <br /> desirous of having its students receive experience at Orange County Health Department <br /> beginning on or about July 1, 2021 until August 30, 2023 or until such time either party desires <br /> to terminate contract by giving a 30-day written notice; and <br /> WHEREAS, the Orange County Health Department wishes to cooperate by working with the <br /> faculty and students of the School of Public Health in permitting the use of its facilities in the <br /> ways hereinafter set forth; and <br /> WHEREAS, both parties anticipate that two master's degree candidates will participate for up to <br /> Ten-weeks (400 hours); <br /> NOW THEREFORE, in consideration of the premises and the following mutual promises, the <br /> parties agree as follows: <br /> A. The University of North Carolina at Chapel Hill for its School of Public Health agrees <br /> 1. To be responsible for the following services that will be provided through the faculty <br /> liaison of the Department of Nutrition: <br /> a. supplying curriculum and course information to a designated Orange County Health <br /> Department(hereinafter referred to as Agency) representative; <br /> b. communicating periodically with the designated Agency Representative to: <br /> 1) discuss the nature of the students' learning experiences <br /> 2) provide for continuity of dietetic service, and <br /> 3) monitor the quality of the services and products provided by the students; <br /> c. evaluating the learning experiences of the students; <br /> d. planning and conducting student conferences; <br /> e. working closely with the designated Agency representative and staff to maintain and <br /> promote the policies and services of the Agency; <br />
