ORANGE COUNTY HEALTH DEPARTMENT <br /> Board of Health Policy and Procedure Manual <br /> Section I: Board Adopted Policies <br /> Policy C: Community Assessment Policy <br /> Reviewed by: Board of Health <br /> Approved by: Board of Health,Health Director <br /> 1. The CHA document will be used by the Orange County Health Department in the <br /> development of the department-wide strategic plan, grant writing, program <br /> planning and advocacy for funding. This document will be available as a resource <br /> for other individuals, agencies, and organizations. <br /> m. Using the CHA, Health Promotion and Education staff and the CHA team will <br /> create Community Health Action Plans to describe plans for health activities to be <br /> carried out in the county. The Community Health Action Plan form is due the <br /> first Monday in September following the year of assessment. The form is <br /> available through the NC Division of Public Health website. <br /> 2. State of the County Health(SOTCH)report process procedures: <br /> a. The SOTCH report will primarily be produced by the Department's Health <br /> Promotion and Education staff in the interim years between community health <br /> assessments. <br /> b. This report will include: <br /> i. A review of major morbidity and mortality data for the county <br /> ii. A review of health concerns selected as priorities <br /> iii. Progress made in the last year on these priorities <br /> iv. A review of any changes in the data that guided the selection of these <br /> priorities <br /> v. Other changes in the county that affect health concerns (such as <br /> economic or political changes, new funds or grants available to <br /> address health problems, etc.) <br /> vi. New and emerging issues that affect health status <br /> vii. A review of disaggregated data by race, ethnicity, gender, sexual <br /> identity, etc. to account for health disparities in the county among <br /> marginalized groups. <br /> viii. Methods of direct community involvement with ongoing efforts <br /> c. The primary source of data for this report will be the NC SCHS website. <br /> d.
