ORANGE COUNTY HEALTH DEPARTMENT <br /> Board of Health Policy and Procedure Manual <br /> Section I: Board Adopted Policies <br /> Policy C: Community Assessment Policy <br /> Reviewed by: Board of Health <br /> Approved by: Board of Health,Health Director <br /> d. The Community Health Assessment Guide Book available on the North Carolina <br /> Division of Public Health website will be used as a resource document or toolkit <br /> throughout the community health assessment process. This book will guide the <br /> team on the various components (i.e. demographics, economic factors, health <br /> needs, etc.)that need to be included in this process and the final document. <br /> e. Primary data will be collected from community members, clients receiving direct <br /> services from the health department, the Board of Health and other county <br /> stakeholders through their participation in written surveys, focus groups, or <br /> interviews. <br /> f. Secondary data will be obtained from the North Carolina State Center for Health <br /> Statistics (NC SCHS) and other resources identified and available to CHA team <br /> members. <br /> g. CHA team members, OCHD staff, and community volunteers will assist with the <br /> collection of primary and secondary data. <br /> h. Primary and secondary data collected should be disaggregated by race, ethnicity, <br /> gender, sexual identity, etc. when possible to account for racial and health <br /> disparities among marginalized groups. <br /> i. The CHA team will collaborate with county stakeholders to prioritize health <br /> concerns according to the primary and secondary data collected. The CHA team <br /> will reference the CHA Guidebook for guidance on reporting data findings and <br /> involving community members, the Board of Health and other county <br /> stakeholders in the process to establish health priorities for the county. <br /> j. Designated members of the CHA team will summarize the data and priority health <br /> topics to produce a document to report the community health assessment process <br /> and its findings. <br /> k. The CHA document will be submitted to the North Carolina Division of Public <br /> Health by first Monday in March following the year of assessment and will be <br /> disseminated to community and county stakeholders as specified in the North <br /> Carolina Local Health Department Accreditation Standards.
