With grant funding, staff have been able to use innovative <br /> approaches to increase access to these services and to expand community <br /> partnerships with the goal of improving health outcomes. As a result of these <br /> collaborations and funding opportunities, Nutrition Services now offers the CDC minority <br /> diabetes prevention program (MDPP) and employee wellness services. <br /> With funding from the Office of Rural Health (9/17, renewed FY19-21), OCHD was able <br /> to implement several initiatives to increase access to primary care and support services: <br /> A nutrition services voucher program (FY19-20, 228 vouchers); Enhanced interpretation <br /> support (4 Spanish diabetes classes); Diabetes testing supplies/resources; And a <br /> transportation assistance program (FY19-20, 45 rides). The grant also advanced <br /> community partnerships (Dept. on Aging, CCPHD, UNC Family Medicine). <br /> With COVID-19, Nutrition Services initiated telehealth services (12% decreased no <br /> show rate), developed protocols to disseminate self-management tools, facilitated <br /> remote MDPP services (Average 6.6% weight loss and 154minutes PA/week), and <br /> continued individual and group employee wellness services (FY19-20, 100 encounters). <br /> Recommended Action: Approve <br /> Approve & forward to Board of Commissioners for action <br /> Approve & forward to <br /> _X_Accept as information <br /> Revise & schedule for future action <br /> Other (detail): <br />
