04/14/2020 <br /> families upon graduation or completion from the <br /> Family Navigator program. The FSA Connections <br /> program allows FSA to maintain contact with families <br /> formerly assigned to a navigator as well as to provide <br /> services to those families on the Family Navigator <br /> waitlist to ensure that needs are addressed pending a <br /> navigator assignment. <br /> • NC Cares360: We are excited to now be a part of the <br /> statewide NCCARE360 care management system <br /> and see this as a way to improve our ability to <br /> connect FSA families to the services they need <br /> through warm handoffs and follow-ups while also <br /> accurately capturing the data we need to show <br /> success and challenges. <br /> • Kindergarten Readiness: FSA has sponsored a <br /> Kindergarten Readiness camp in each zone for 5 <br /> years. To date, 378 rising kindergarteners have <br /> participated in the program. As a result of receiving <br /> Oak Foundation funds, Chapel Hill Carrboro City <br /> schools will extend their Ready to K program for 60 to <br /> 140 students. <br /> • Partner Network: FSA coordinates a partner network. <br /> We recently expanded the network from 10 to 16 <br /> partners. We use the network to center parent <br /> expertise and priorities as we problem-solve and work <br /> towards outcomes for families. We aim to provide a <br /> space for natural partnership and accountability to <br /> assist each other in identifying strengths and gaps in <br /> service provision. <br /> • Community Council: FSA has re-tooled the Parent <br /> Council and it is now called the Community Council. <br /> Navigators worked together to recruit parent <br /> representatives from larger communities from both <br /> zones. The council currently consists of 10 FSA <br /> parents and other individuals of specific communities <br /> who are currently active or have a strong interest in <br /> advocating and making changes for and with their <br /> neighbors. It is FSA's mission to learn from these <br /> parents about the strengths and needs of each <br /> community that FSA will be serving and to also <br /> provide information and resources to these parents in <br /> a way that can be redistributed by these parents to <br />
