Agenda Item Number: <br /> ORANGE COUNTY BOARD OF HEALTH <br /> AGENDA ITEM SUMMARY <br /> Meeting Date: March 25, 2020 <br /> Agenda Item Subject: Nutrition Services Update <br /> Attachment(s): <br /> Staff or Board Member Reporting: Renee Kemske, Nutrition Program Manager <br /> Purpose: Action <br /> X Information only <br /> Information with possible action <br /> Summary Information: The Nutrition Services section has historically offered medical nutrition <br /> therapy and diabetes self-management education services to residents. With grant funding, <br /> staff have been able to use innovative approaches to increase access to these services as well <br /> as expand community partnerships to improve health outcomes. <br /> OCHD was initially awarded funding (9/17) for a Community Health Grant from the Office of <br /> Rural Health to increase access to primary care and self-management services for adults with <br /> chronic conditions. OCHD piloted a voucher program covering nutrition services fees, provided <br /> diabetes testing supplies and offered diabetes classes in Spanish. With UNC Family Medicine, <br /> OCHD initiated a Community Health Collaborative to increase awareness of local resources and <br /> to formalize referral systems. OCHD was awarded additional funding (7/18: 3 years) to expand <br /> programing efforts: Develop a transportation assistance program; replicate the voucher <br /> program in Chatham County; and increase collaboration with the Department on Aging. <br /> OCHD joined a nine-county collaborative to offer the CDC Minority Diabetes Prevention <br /> Program, have held three cohorts and is now a fully recognized CDC program. The last cohort <br /> exceeded 5% weight loss and 150 minutes of physical activity per week. Additionally, the <br /> agreement for County wellness funds to provide free nutrition services (individual and group) <br /> has been well received and specific departments are requesting customized services. <br /> Recommended Action: Approve <br /> Approve & forward to Board of Commissioners for action <br /> Approve & forward to <br /> _X_Accept as information <br /> Revise & schedule for future action <br /> Other (detail): <br />
