"Secure attachment" includes rivets, stainless fasteners that do not cause <br /> contamination hazards caused by drilling through the casing; and adhesive <br /> intended for use with the casing material. In the case where an electrical <br /> grounding block is secured to the casing by means of a bolt or screw which <br /> penetrates the casing, the tag may be secured by the same means as the block, <br /> provided that no data are obscured. No form of tape by itself, in the absence <br /> of one of the approved methods listed above, shall be considered as a secure <br /> form of attachment. The means of attachment shall not obscure any of the <br /> required data. Secure attachment is not any form of tape alone. <br /> 5. The identification plate shall not be removed by any person. <br /> 6. The identification plate shall be stamped with permanent legible markings to <br /> show the: <br /> a. Name and certification number of the well contractor; <br /> b. Total depth of well; <br /> c. Casing depth (FT) and inside diameter(in.); <br /> d. Screened intervals of screened wells; <br /> e. Packing interval of gravel-or sand-packed wells; <br /> f. Yield, in gallons per minute (gpm), or specific capacity in gallons per <br /> minute per foot of drawdown(gpm/FT-dd); <br /> g. Static water level and date measured; <br /> h. Date well completed; and <br /> i. Well Permit number or numbers, if such a permit is required. <br /> History Note: Substitute for NCAC 02C.0107 <br /> Eff. July 1, 2008;Amended eff. October 23, 2009;March 25, 2020 <br /> L. PUMP INSTALLER INFORMATION PLATE <br /> 1. For all wells except monitoring wells from Section III (B) of these Rules, an <br /> identification plate shall be installed on the well within 72 hours after <br /> completion of the pump installation; <br /> 2. The information plate shall be constructed of a durable waterproof, rustproof <br /> metal or equivalent material approved by the OCHD. <br /> 3.
