a. A map of the general site area, showing the location of: <br /> (1) All property boundaries, at least one of which is referenced to a <br /> minimum of two landmarks, such as identified roads, intersections, <br /> streams or lakes; <br /> (2) All existing wells, identified by the type of use, within the <br /> property boundaries; <br /> (3) All proposed wells, identified by type of use, within the <br /> property boundaries; and <br /> (4) All sources of known or potential groundwater contamination <br /> within the property boundaries. <br /> b. A construction profile diagram of the proposed monitoring well or <br /> injection well showing the type of well and including detailed <br /> specifications describing all materials to be used and methods of <br /> construction. <br /> c. The well contractor company name, if known, an <br /> d. Unless the well is deemed permitted per 15A NCAC 02C, an approval <br /> letter from the State indicating review and approval of the proposed <br /> well(s). <br /> 11. An application for a closed-loop geothermal well shall also include: <br /> a. The type of recirculation fluid or gas, and any additives to be used. <br /> Documentation from the State must be provided if the fluid or additive <br /> is not pre-approved, <br /> b. A detailed site plan showing the locations of the well or wells and <br /> the horizontal piping routes, and, <br /> c. A detailed profile diagram showing the components of the geothermal <br /> well system. <br /> d. Any current or pending restrictions regarding groundwater use as <br /> specified in the law and these Rules. <br /> History Note: Substitute for NCAC 02C.0303 <br /> Eff. July 1, 2008;Amended eff. March 25, 2020 <br /> A PERMITTING <br /> 1. No person shall construct a private drinking water well without first obtaining <br /> a well construction permit from the Orange County Health Department. No <br /> person shall repair a private drinking water well without first obtaining a well <br /> repair permit, except a well repair permit is not required for maintenance or <br /> pump repair or replacement. Disinfection in accordance with Section II of <br /> these Rules is a maintenance activity that does not require a repair permit. No <br /> person shall permanently abandon a private drinking water well without first <br /> obtaining a well abandonment permit from the Orange County Health <br /> 12 <br />
