(1)Well contractor name and certification number; <br /> (2)Number and depth of the boring(s); <br /> (3)Grout depth interval; <br /> (4)Well construction completion date; and <br /> (5) Identification as a geothermal well/well field; or, <br /> b)The perimeter corners of each well field shall be marked at ground surface <br /> with a permanent monument that: <br /> (1) Identifies the area as a geothermal well field, and; <br /> (2) Refers to the location of a recorded plat prepared by a Registered <br /> Land Surveyor that identifies the location, depth and date of each well. <br /> The plat shall be recorded at the Orange County Register of Deeds and <br /> a copy shall be retained by the property owner and the OCHD. <br /> (3)The property owner is responsible for compliance with all other <br /> well identification requirements contained in 15A NCAC 02C <br /> .0222(f) and .0223(f) <br /> History Note: Substitute for NCAC 02C.0108 <br /> Eff. Every well shall be maintained by the owner in a condition whereby it will conserve <br /> and protect the groundwater resources, and whereby it will not be a source or <br /> channel of contamination or pollution to the water supply or any aquifer, or the well <br /> shall be permanently abandoned in accordance with the requirements of these Rules <br /> and this Section. An electrical box is required for wiring <br /> connected at the well head. <br /> 2.All materials used in the maintenance, replacement, or repair of any well shall meet <br /> the requirements for new installations. <br /> 3. Broken, punctured or otherwise defective or unserviceable casing, screens,fixtures, <br /> seals, or any part of the well head shall be repaired or replaced, or the well shall be <br /> permanently abandoned pursuant to the requirements of this Section. <br /> 37 <br />
