2.The information plate shall be constructed of a durable waterproof, rustproof metal or <br /> equivalent material approved by the OCHD. <br /> 3.The identification plate shall be securely attached to the aboveground portion of the <br /> well casing where it is readily visible and in a manner that does not obscure the <br /> identification plate. <br /> 4. No form of tape by itself, in the absence of one of the approved methods <br /> listed above,shall be considered as a secure form of attachment. The means of <br /> attachment shall not obscure any of the required data. Secure attachment is not any <br /> form of tape alone. <br /> 5.
