b)All property boundaries, at least one of which is referenced to a minimum of <br /> two landmarks, such as identified roads, intersections, streams or lakes; <br /> c)All existing wells, identified by the type of use, within the property <br /> boundaries; <br /> d)All proposed wells, identified by type of use,within the property boundaries; <br /> e)All sources of known or potential groundwater contamination within the <br /> property boundaries. <br /> f)A construction profile diagram of the proposed monitoring well or injection <br /> well showing the type of well and including detailed specifications describing all <br /> materials to be used and methods of construction. <br /> g)The well contractor company name, if known, and <br /> h) Unless the well is deemed permitted per 15A NCAC 02C, an approval letter <br /> from the State indicating review and approval of the proposed well(s). <br /> i)An application for a closed-loop geothermal well shall also include: <br /> j)The type of recirculation fluid or gas, and any additives to be used. <br /> Documentation from the State must be provided if the fluid or additive is not pre- <br /> approved, <br /> k)A detailed site plan showing the locations of the well or wells and the <br /> horizontal piping routes, and, <br /> 1)A detailed profile diagram showing the components of the geothermal well <br /> system. <br /> m)Any current or pending restrictions regarding groundwater use as specified in <br /> the law and these Rules. <br /> History Note: Substitute for NCAC 02C.0303 <br /> Eff. PERMITTING <br /> 1.b No person shall construct a private drinking water well without first obtaining a well comment[VH29]:SAME AS BEFORE;SECTION <br /> construction permit from the local health department.
