OTHER SUBMITTAL INFORhIATION: <br />Elevations of all structures proposed to be used is the development. <br />~ Two (2) full-size copies of the applicable Orange County Tax Map, one (1) copy <br />with the property in question clearly marked. <br />'The names and addresses of the property owner(s) and/or applicant(s), and the <br />names and addresses are all persons owning property within five ituadred <br />~ (S00) feet of the property in question. <br />Application fee as set by the Orange County Hoard of Commissioners. <br />rTraf fic impact study as required by Article 13 of the Zoning Ordinance. <br />Additional information rcgardiag the proposed Special Use as required by <br />Article 8 of the Zaniag Ordinance. <br />Narrative (or letters from appropriate agencies) indicating: <br />1. Method and adequacy of provision of sewage disposal facilities, solid <br />waste disposal, and water service. Where public sewer is not <br />available, a letter from the Orange County Health Department <br />certifying the suitability of the existing and/or proposed <br />wastewater treatment system for the property. <br />2. Method and adequacy of police, fire, and rescue squad protection. <br />3. Method and adequacy of vehicular access to the site and traffic <br />conditions around the site. <br />I (We), the applicant(s), hereby certify that the foregoing application and supporting <br />documentation is complete and accurate.
