Emsar (hereinafter <br /> referred to as the "Company") and the "Customer" whose name and location of principal offices <br /> are listed on Page 3 of this agreement. <br /> 1. Maintenance Coverage <br /> The Company agrees to provide comprehensive preventive maintenance and repair <br /> services on the equipment of the Customer. This equipment is not limited to FDA <br /> Controlled Class II Medical Devices. The equipment can be Stryker EMS, and others. <br /> 2. Preventive Maintenance Service <br /> 2.1 Definition <br /> Preventive Maintenance shall consist of those routine maintenance services which <br /> are intended to keep the Equipment in proper operating condition in accordance <br /> with the original approved manufacturer's recommendations. <br /> 2.2 Frequency <br /> A scheduled preventive maintenance inspection shall be performed on each <br /> stretcher, Power Load, Performance Load, and stair chair one (1)time. <br /> 2.3 Scope <br /> During the inspection, the equipment will be hand washed, disinfected, lubricated <br /> and inspected for damage or wear. Cleaning is limited to no more than 20 minutes <br /> per unit. <br /> 2.4 Additional Repairs <br /> If damage or wear is detected during the preventive maintenance inspection, it will <br /> be noted on the Equipment Report and brought to the attention of the Customer. <br /> Fastener rails are inspected and adjusted at no additional charge. New fastener <br /> rails are available for installation at the time of service. Repairs to the fastener rail <br /> system will be billed as additional labor and parts. Note: Repair service for the <br /> stretchers and stair chairs includes parts and labor with exceptions noted in <br /> Addendum 1. <br /> 2.5 Documentation <br /> An Equipment Report shall be completed by the technician on each piece of <br /> equipment serviced. This report shall document the Customer, serial number, <br /> date, location of service, and identity of the service technician. It shall also specify <br /> which parts were replaced or repaired. Repairs and evaluation are made in <br /> accordance with proprietary and copyrighted Stryker technical manuals. <br /> 2.6 Lubrication <br /> Stryker equipment is lubricated with factory approved lubricants. <br /> 2.7 Hardware <br /> Only new genuine Stryker Parts are used in repairs. No used, modified or <br /> homemade parts will be installed. All new original parts will have met FDA <br /> approved testing standards. This practice will insure your equipment is not <br /> modified. Your equipment will be treated as if there is an FDA Enforcement Group <br /> on site. <br /> Pagel of 3 <br />
