DocuSign Envelope ID: F716EB15-419E-4F35-93B0-ED3DEBFC701A <br /> 2 <br /> £ requiring students to observe policies, rules, and regulations of the Agency including <br /> those related to the protection of confidential health information. <br /> g. providing documentation indicating that each student has professional liability <br /> insurance in the amount of$1,000,000 per occurrence and$3,000,000 aggregate. <br /> g. completing a criminal background check on each student. <br /> h. assuring that all students complete online HIPAA training and OSHA Bloodbome <br /> Pathogen training prior to their field experience. <br /> i. requiring students to comply with any requirements of Agency regarding <br /> immunizations. Any immunizations required which student does not yet have must be <br /> obtained prior to beginning student's experience and the cost for the immunization <br /> shall be paid for by student or student's own medical coverage. <br /> 2. To terminate an individual student's experience if the learning activity is shown to be <br /> detrimental to client welfare. <br /> 3. To require students to carry individual health insurance. <br /> 4. To inform students and faculty that they must be responsible for their own transportation <br /> including parking while involved in the learning experiences associated with the Agency. <br /> 5. The Department of Nutrition in the School of Public Health reserves the right to <br /> terminate the experience should the Agency's site coordinator become unavailable or <br /> unable to continue supervising the student(s) due to unforeseen circumstances. <br /> 6. The University of North Carolina at Chapel Hill for its School of Public Health shall <br /> indemnify, hold harmless and defend Agency, its officers, directors, employees and <br /> agents from and against any and all losses, liabilities, claims, damages, causes of action, <br /> costs and expenses, including reasonable attorney's fees and litigation costs arising from <br /> University's breach of any covenants or obligations set forth in this Agreement. This <br /> obligation shall survive the termination of this Agreement. <br /> B. Orange County Health Department agrees: <br /> 1. To designate an appropriate Agency representative to be responsible for maintaining <br /> liaison with the School of Public Health. In this connection the responsibility of this <br /> person will be: <br /> a. to supply information concerning the Agency, staff, and clients (including <br /> health/medical records of the latter)that will facilitate the students' learning <br /> experience(s). It is understood that protected health information will be kept <br /> confidential. <br /> b. to communicate periodically with the faculty to: <br />
