DocuSign Envelope ID:2E2387F3-3A58-44C5-A8FF-30C659A8AB71 <br /> Attachment A <br /> Lake Orange - Sluice Gate Stem Guide Fabrication and <br /> Installation - Scope Description <br /> The existing Lake Orange intake tower drain gate (48-inch wide steel sluice gate) and the associated <br /> manually-operated top-slab mounted actuator are believed to be original to the initial construction of <br /> Lake Orange in 1969. The existing sluice gate and actuator were fabricated by Rodney Hunt, Inc. <br /> Currently, the existing sluice gate is very difficult to operate manually. An inspection conducted in <br /> September 2018 revealed the presence of sediment and/or corrosion within and around the sluice <br /> gate tracks, the sluice gate stem guides, and the sluice gate seating surface. <br /> In March 2020, In-Water Services performed cleaning/de-mucking work on the intake tower drain <br /> gate under previous contract with Orange County. While performing this de-mucking work, In-Water <br /> Services confirmed that the intake tower drain gate is completely inoperable due to corroded/non- <br /> functioning sluice gate stem guides and wall mounts. New sluice gate stem guides must be fabricated <br /> and installed to ensure that the intake tower drain gate is fully operable at all times. <br /> Scope Description: <br /> No utility location is required. No permitting is required. This work shall be conducted using OSHA <br /> confined space procedures and will require the support of a standby rescue team. The standby <br /> rescue team will be coordinated and provided by Orange County. The project site is located entirely <br /> within Orange County property. The project site address is 1221 Lake Orange Road, Hillsborough, <br /> NC 27278. There is a 120V, 20A power source (outlet box) on site. See Attachment B for the <br /> location of the project. See Attachment C for photographs of the proposed work area and an <br /> example of the required sluice gate stem guides. Detailed scope is as follows: <br /> A. Provider shall supply all labor and materials for the custom fabrication (or modification of <br /> compatible stock materials) of four (4) sluice gate stem guides for the Lake Orange intake <br /> tower drain gate. Sluice gate stem guides shall be painted carbon steel, roughly 5/8-inch <br /> thick, with 3.75-inch stem orifices and shall be delivered to the Lake Orange site by the <br /> Provider for subsequent installation. <br /> B. Provider shall supply all labor and materials for the removal of the four (4) existing sluice <br /> gate stem guides and the installation of the four (4) new, custom fabricated sluice gate <br /> stem guides for the Lake Orange intake tower drain gate. Provider shall coordinate this <br /> work in the field with Orange County staff. <br /> C. Provider shall supply all labor and materials for operation/testing of the four (4) newly <br /> installed sluice gate stem guides so as to ensure the full operability of the Lake Orange <br /> intake tower drain gate, including but not limited to: <br /> Page 1 of 2 <br />
