Does the facility have a Resident’s Council? <br />Family Council? <br />Yes <br />No <br /> <br /> Areas of Concern Yes/No/NA Exit Summary <br />Are there resident issues or topics that need follow-up or review at a later <br />time or during the next visit? <br />• Staff Courtesy <br />• Food choices <br />• Opportunities for activities <br />• Nail care <br /> <br />Committee members found evening shift nursing supervisor to be highly <br />accepting of care concerns. <br /> <br />Also noted was an easily read colorful video bulletin board listing resident <br />rights and ways in which concerns and thanks could be made to <br />administration. <br />Yes Courtesy: One family member sought out <br />committee members; very distressed about trying <br />to help spouse late at night via telephone call to <br />staff. Spouse asking to use urinal; staff response <br />heard by wife was “we’re going to put you in a <br />diaper” when resident asked for assistance with <br />urinal. Spouse tried a second time to reach staff <br />for assistance; call answered by different staff <br />member and when spouse asked: “to whom am I <br />speaking;” staff responded in perceived unhelpful <br />manner “you don’t need to know;“ Nursing <br />supervisor said she would immediately address <br />after meeting. Committee members also provided <br />Ombudsman Information to family member. <br /> <br />Food Choices: Often voiced as either overcooked <br />or unheated. Continues as a concern <br /> <br />Activities: When resident who was reading a <br />book; would have like some type of library to <br />make own choices. <br /> <br />Nail Care: Facility provides manicures; resident s <br />have right to decline however based on response <br />to visitor’s questions about nail length, it appeared <br />as if this need had not be appropriately <br />addressed. <br />This Document is PUBLIC RECORD.
