Are there resident issues or topics that need follow-up or review at a later <br />time or during the next visit? <br /> <br />1/ The seating options in the Memory Care Unit were grossly limited and <br />inadequate for this population, many of whom were wheelchair bound. Most <br />chairs had no arms and little padding, only two recliners were available, and <br />those who used wheelchairs were all seated in them, though many were <br />flimsy lightweight foldup transport chairs with no cushions. <br />2/ The building interior needs lots of TLC. Paint has been chipped in <br />the hallways and rooms, there are old water stains and cracks in <br />ceilings, and smudges abound. <br />3/ There were some faint residual odors of urine, dirty clothes, <br />chemicals but staff were working diligently while we were there. <br />During our exit conference with the new Director, she addressed all <br />issues well. She is ordering all new dining room furniture for the <br />Memory Unit and the handyman is working to correct building issues. <br />She is hoping for less staff turnover and said she is there for the long <br />haul. <br /> <br />Y Discuss items from “Areas of Concern” Section as <br />well as any changes observed during the visit <br /> <br />Activity Director, Shylea Byrd, seems to <br />have settled in. She has a variety of <br />activities planned, both in the main unit and <br />in the memory-care unit. She continues <br />even though she knows she may have very <br />low attendance. <br />The new (4 months) director, Lisa, acknowledges <br />the shortcomings in the facility. She is an <br />experienced Stratford Corp. employee and seems <br />to be ready for the task. Auditors are coming soon <br />and changes are planned for furniture and lights. <br /> <br /> <br /> <br />This Document is PUBLIC RECORD.
