16 <br />The Nutrition and Aging Director and Coordinator of Services aze responsible for the day-to day <br />Supervision of the Congregate Meals in Orange County. In cooperative agreements with the <br />Orange County Department on Aging, UNC School of Nursing, local Health and Recreation <br />Departments, Cooperative Extension, North State Legal Services, Durham Technical <br />Community College, and the N.C. Division of Aging, JOCCA is able to provide but nat limited <br />to the following program services to its participants: <br />Creative Arts and Crafts including Games <br />Nutrition/Health Education Classes <br />Assessments of Vital Signs <br />Preliminary Health Screening-e.g. Blood Pressure and Diabetic Checks <br />Legal Services <br />Adult Basic Services (ABE) <br />General Equivalency Diploma (GED) <br />Annual May Day Programs <br />Health Fairs <br />Crime Prevention Workshops <br />Rape Prevention Workshops <br />Transportation to Congregate Nutrition Sites <br />Shopping and Recreational Trips <br />The Congregate Nutrition Luncheon Program is advertised in the local newspapers. JOCCA's <br />brochures aze also disseminated through out the service delivery area. Announcements aze <br />also made at all interagency meetings about the luncheon program. <br />Referrals for program participation are received from cognate human service agencies, <br />community residents, local community churches, civic organizations and current program <br />participants. <br />Priority consideration for service (s) will be given to older individuals who exemplify the <br />geatest socioeconomic need; special attention will be given to low-income minority <br />Individuals. The following priority system will be utilized. <br />(1) Highest priority will be given to the abused, neglected and /or exploited and for wham <br />the service is needed as identified by DSS; <br />(2) Individuals at risk of abuse, neglect and/or exploitation; <br />(3) Individuals who do not have a care provider or another responsible party to assist with care; <br />(4) Individuals who experience impairment in performance of daily living. <br />
