Housing Access Coordinator (HAC) <br /> Program Overview <br /> An evidence-based practice that will increase Orange County residents' livelihood security <br /> and access to basic services.The HAC will fill the gap in the homelessness services system by <br /> increasing affordable units available through private landlords, developing tools to mitigate <br /> barriers and locate housing, and supporting at least 120 county residents in their housing <br /> search.The HAC will partner with HOME Taskforce and Orange County Affordable Housing <br /> Coalition, collaborating with 50 social services provider and housing-focused agencies, to <br /> support housing seekers and collect data on the unmet housing needs in Orange County. <br /> New Projects <br /> The HAC will grow a partnership with the Orange County Department of Housing and <br /> Community Development, including training Housing Choice Voucher (HCV) staff on <br /> housing navigation tools and exploring methods to track the number of program participants <br /> outside of CEF who obtain housing or utilize the Risk Mitigation and Housing Displacement <br /> Fund.As part of this,the HAC will collaborate with the Orange County Housing Authority <br /> (OCHA)to schedule a landlord roundtable to establish a feedback loop for the HCV program, <br /> address feedback, and educate landlords about the benefits of participating in the HCV and <br /> other subsidy programs. <br /> Program Deliverables <br /> Success in this initiative will be measured by the following: <br /> Performance Measurement Quantifiable Objective rY 2019-20 <br /> Projected <br /> Outcome <br /> Assessed the housing status upon intake and %and#of program participants who 70 <br /> document changes in housing status, determined are homeless or experiencing unstable <br /> through regular updates and continued follow-up housing who obtain housing <br /> with members <br /> Track individuals who participate in housing support %and#of individuals who receive 120 <br /> services through the Housing Locator housing support information,services <br /> or referral <br /> Track meetings,outreach and relationship building #of Landlords engaged by the Housing 20 <br /> moments with existing and new Landlord partners Locator <br /> Community Empowerment • • • 11 1www.communityef.org <br />
