1
Exhibit A
Scope of Services for
Northside Elementary After School 2019-2020 program
for the Family Success Alliance

Purpose: Program support resources for Northside Elementary to provide after school
programming for students attending NES, a zone 6 school.

Budget: $2,500.00
x Funds will be used for teacher salaries for the program. Total program budget is $3,600
with funds leveraged from other sources.

Program Services:
x The school will offer a Tier III after-school HillRAP reading program (from the Hill School)
for two groups of four students in grades 2-3 for a 12 week time period.
x The goal of the program is to grow the reading skills of the participants so they can
better access grade level materials and hopefully, learn to enjoy reading rather than
struggle through it.
x The Reading Interventionists, Exceptional Education teachers, and ESL teacher have
chosen students in third grade who will be offered a place in the program. These
teachers have assessed the students using CORE Phonics Survey and Beginning of Year
data. The teachers also considered students’ learning styles. Teachers have matched
the learning needs and styles of the students to the program so that students who are
most likely to respond and grow from the program will participate. Additionally, the
students identified as those from households deemed to be in financial need will be
given priority in the program.
Outcomes
x Students will keep track of their own growth through the daily formative assessment.
They will know their target for growth and their progress. They will chart their progress.
Each student group will be comprised of one teacher and no more than 4 students. This
small group size ensures the students actively participate. They are engaged the entire
time.
Reporting
x Half of the contract will be paid when the contract is signed and executed. The
remaining half will be paid when the program is complete and a final invoice and
program report are submitted. The deadline to submit these documents is May 15,
2020.

DocuSign Envelope ID: 1A17460C-785F-4B40-A40A-4C0474E40E4D
