Consolidated Agreement -Final <br />CONSOLIDATED AGREEMENT <br />BETWEEN <br />THE STATE OF NORTH CAROLINA <br />AS REPRESENTED BY <br />THE STATE HEALTH DIRECTOR <br />(Hereinafter called the "State") <br />AND <br />z <br />Qrange County Health Department <br />(Local Health Department/District/Public Health Authority -- Hereinafter called the "Department") <br />FOR THE PURPOSE OF <br />MAINTAINING AND PROMOTING THE ADVANCEMENT OF <br />HEALTH IN NORTH CAROLINA <br />This Agreement Shall Cover a Period From <br />July 01, 2001 to June 30, 2002 <br />and shall remain in force until the next Fiscal Year Agreement <br />is signed except as provided for in Section J. Termination. <br />NOW, THEREFORE, the State and the Department agree that the provisions and clauses <br />herein set forth shall be incorporated in and constitute the terms and conditions applicable for <br />activities involving State funding. (State funding or funds means state, federal, and/or special <br />funding or funds throughout this agreement.) <br />
