DocuSign Envelope ID:5FA4B1AC-B897-4C32-A1E4-4B4A8885A341 <br /> rROWDER CrowderGulf E <br /> LILULF RFP for Disaster DebrisClearance and Removal Services <br /> Orange County, NG <br /> Truck Certification <br /> All debris hauling trucks will be certified by the County or a County representative before any hauling begins.The inside <br /> bed dimensions of all trucks will be accurately measured and all safety requirements will be checked and approved. <br /> This information along with the description and a picture of the truck, driver's name, license and tag number will be <br /> recorded on the FEMA compliant certification forms provided by CrowderGulf, The County will retain the original copy <br /> of the form and provide CrowderGulf and the driver with copies.The driver's copy must remain in the truck at all times. t <br /> A placard displaying the trucks identification and measurement information will be displayed on both sides of the vehicle <br /> at all times. Specific truck documentation requirements are discussed in the Documentation and Reimbursement <br /> Section of this proposal. <br /> Sectioning and Crew Assignments <br /> Upon NTP, CrowderGulf will assist Orange County in assessing damage and developing a specific plan of action.The <br /> affected areas will be divided into sections and then crews,subcontractors, and equipment will be assigned.All areas <br /> will be served simultaneously. <br /> Loading Debris <br /> Prerequisites for Loading Debris; <br /> • Truck certification and safety inspections completed on all trucks hauling debris <br /> • Identification of disposal site <br /> • if needed for efficiency, identification and preparation of debris management sites <br /> • Sectioning of County with subcontractor assignments <br /> ■ Quality Control organization operational <br /> Load ticketing and data management process operational <br /> Accident Prevention Plan (APP), Site Safety and Health Plans (SSHP), Activity Hazard Analyses(AHAs) <br /> • Initial safety and health briefing for all personnel complete <br /> • Specific training on traffic control complete for all debris crews <br /> ■ Preparatory inspections of each worksite by Quality Control staff and debris crew foreman <br /> ■ Work area cleared of safety and health hazards such as downed power lines and hazardous materials <br /> Inspection of work area for water meters, fire hydrants, utility pedestals and other infrastructure components that <br /> could be damaged by equipment <br /> • FEMA debris eligibility criteria communicated to all Crew Foremen, Superintendents, Quality Control staff, Project <br /> Managers,and Equipment Operators <br /> • Overhead power lines and other utility fines identified for safe clearance of loading equipment <br /> Crew Composition <br /> CrowderGulf matches equipment to the requirements of the task. Crew composition varies depending on the type of <br /> equipment used in performing the loading operation.For example,crews with self-loading trucks do not need separate <br /> loading equipment that is required to support a crew consisting of dump trucks or trailers. However,every crew requires <br /> traffic control personnel, a foreman and a designated quality control person. Also, each crew requires a chain saw <br /> operator and laborer(s) to assist in the ground support work. Usually each piece of loading equipment (self-loading <br /> truck or separate loader) is supported by one saw operator, two laborers and two fiaggers. At a minimum, debris <br /> separation crews will consist of two laborers,one chain saw operator with saw,one skid steer loader with operator and <br /> implements, all equipment necessary to transport personnel and equipment from one work site to another. When <br /> necessary,ground crews will separate and sort the debris by type,saw fallen trees and vegetative debris at the public <br /> rights-of-way, and be constantly alert for water meters, fire hydrants, utility pedestals and other infrastructure <br /> components that could be damaged by equipment. <br /> Crew Sizes <br /> The size of debris loading crews will be dictated by the severity and localization of damage. Each crew foreman will he <br /> experienced in organizing and directing debris crews and will be provided with sufficient chain saw operators,flagmen, <br /> laborers and knuckle boom operators to assure rapid and efficient debris removal. <br /> An example of a Crew is demonstrated in the chart below; <br /> 101 <br />
