Yes No. If yes, the amended date of subs completion is: February 14, 2020 ___________________________________________________________________________________________________ Full Description of Change: This change request includes the following: 2nd Floor layout changes required to meet code including demolition, framing, drywall, painting, mechanical, electrical an changes. Roof Replacement generally described as a re-roof of the existing facility, including alternate to use (2) layers of insulation. Plumbing repairs to lines and fixtures found once drywall was removed, including damages to lines with holes, screw holes, and venting for lines not vented all all and accepting alternater for new shower valve to replace unit downst life, and replacement of all light fixtures with LED lights. Reason for Change: During the demolition it was discovered wooden studs within the walls that are no longer allowed by therefore the wall changes are being made to bring the existing interior walls up to code. This requires additional mechan and fire alarm work that was not planned within the original scope of work since the interior walls were not being remove being replaced due to during construction it was determined that the roof was in a more deteriorated state than previou the roof work started this uncovered other problems. This re-roof will install a new roofing system over the existing roofi which will also increase the insulation value, more energy savings and also noise reduction for the call center. The plumbi required due to finding multiple drain lines and vents with puncture holes from screws used for the original installation o and one vent was not vented outside or tied into an existing system, it was vented directly into the ceiling cavity. This ven the exterior of the building. The shower valve within the shower room is at end of life and to ensure no leaking in the futu replaced at this time. The LED light fixtures are to replace the existing light fixtures. This will support the BOCC goals for e and allows the total number of fixtures to be reduced compared to the number of fixtures originally installed. ___________________________________________________________________________________________________ Original contract sum: $ 1,499,409.00 Contract sum prior to this change order: $ 1,984,524.48 Amount of this change order: $ 312,075.50 Total sum of the contract including this change order: $ 2,296,599.98 ___________________________________________________________________________________________________ This change order is executed to amend the contract time and/or contract sum.
