Name and address of at least three references including contact names <br /> for which the Respondent has performed the same or similar services <br /> within the last three to five years. <br /> See file entitled, "Organizations reaching out with Lifeline". <br /> Evaluation Criteria <br /> Proposals will be reviewed and evaluated on the ability of the Respondent to meet or exceed <br /> the requirements set forth in the specifications. <br /> Determination of the respondent's ability to perform accordingly shall be made by an evaluation <br /> team consisting of representatives from Orange County Health Department, Orange County <br /> Information Technology, Orange County Finance, Orange County Asset Management Services <br /> and Orange County Emergency Services. <br /> Proposals will be reviewed and ranked first on proposal content, then reviewed by proposal <br /> cost, with a final proposal selected based on a combination of content and cost. <br /> Proposals shall be evaluated on the following criteria: <br /> 1. The quality and clarity of the proposal as well as the presentation. (25%) <br /> 2. Ability to meet the required elements of the system. (25%) <br /> 3. Experience, both general and with projects of this type. (15%) <br /> 4. Respondent's approach to training and support. (10%) <br /> 5. Cost (10%) <br /> 6. Quality of past work. (10%) <br /> 7.
