DocuSign Envelope ID:A18808EO-5F70-4494-BCOC-49B95D82162D <br /> Mobile Dental Clinic RFP <br /> RFP#5261 <br /> Orange County Mobile Dental Clinic <br /> Purpose <br /> Orange County (the Department) seeks proposals from qualified and experienced <br /> manufacturers of specialty vehicles for the purchase of one fully equipped mobile dental clinic. <br /> The term of the contract is effective with the execution of the contract through June 30, 2020. <br /> Background <br /> The Orange County Health Department is a public health agency with one dental clinic facility <br /> located in Hillsborough, North Carolina. Services provided include examinations, x-rays, <br /> cleanings, restorations, extractions and anterior root canals. Dental clinic staff include two <br /> fulltime dentists, one general practice resident from UNC School of Dentistry, one temporary <br /> dentist, 3 dental hygienists, 5 dental assistants, 3 office assistants, and one office manager. The <br /> dental clinic serves 500 to 650 patients per month and performs over 2,000 procedures monthly. <br /> The current dental clinic facility is located in the northern part of Orange County. The mobile <br /> dental clinic (hereafter referred to as "unit") will be used to provide preventative and restorative <br /> dental services as well as follow-up care at public elementary schools. The unit will also provide <br /> services in the southern part of the county by parking at the Southern Human Services Center <br /> two days per week and providing services to scheduled patients and emergencies. On the days <br /> the mobile unit is not parked at the facility, it will primarily visit elementary schools with the <br /> highest percentage of children on the free and reduced cost lunch program. The intent is to <br /> provide dental screenings, sealants, and uncomplicated dental procedures while the unit is <br /> parked at the school. When not visiting schools, the goal is to use the mobile unit to visit nursing <br /> homes, assisted living facilities, and community centers where dental services can be provided <br /> on location to older adults in need. <br /> Scope of Work <br /> This Scope of Services will become an integral part of the contract between Orange County and <br /> the Contractor. The Contractor hereby agrees to provide services and/or materials pursuant to <br /> the provisions set forth below. <br /> The primary expected outcomes include but are not limited to the following: <br /> 1. A new, delivered unit with enumerated manufacturer's warranty <br /> 2. Diesel engine, automatic transmission, diesel on-board generator, and backup <br /> generator <br /> 2 1 P a g e <br />
