Contract. This Contract consists of this document and additional documents checked below: <br /> a. For Health Department: <br /> i. ® Health Department Additional Terms and Conditions <br /> ii. ® Business Associates Agreement <br /> iii. ® Condition of Contract Statement <br /> b. For Department of Social Services: <br /> i. M The General Terms and Conditions(Attachment A); <br /> ii. M The Scope of Work,description of services,and rate(Attachment B); <br /> iii. M Federal Certification Regarding Drug-Free Workplace(Attachment C); <br /> iv. M Conflict of Interest(Attachment D); <br /> v. M No Overdue Taxes(Attachment E); <br /> These documents constitute the entire agreement between the Parties and supersede all prior oral or <br /> written statements or agreements. <br /> B. Services <br /> 1. Scope'of Work. <br /> a. This Agreement is for the Provider to furnish the services of qualified interpreters and <br /> translators who speak and or write in English and Spanish to County to provide <br /> interpretation and or translation of those languages to County staff and clients. <br /> b. By executing this Agreement, the Provider represents and agrees that Provider is <br /> qualified to perform and fully capable of performing and providing the services required <br /> or necessary under this Agreement in a fully competent,professional and timely manner, <br /> c.
