("Provider"); <br /> WITNESSETH: <br /> WHEREAS,the County and Provider entered into a Services Agreement dated,February 5,2019,to provide <br /> services to be rendered by Provider to Orange County("Original Agreement"); and <br /> WHEREAS, the County and Provider desire to amend the Original Agreement, while keeping in effect all <br /> terms and conditions of the Original Agreement not inconsistent with the terms and conditions set forth <br /> below. <br /> NOW THEREFORE, for and in consideration for the mutual covenants and agreements made herein, the <br /> parties agree to amend the Original Agreement as follows: <br /> Amend Paragraph 1. a. Scope of Work as follows: <br /> i) This Services Agreement ("Agreement") is for professional services to be rendered by <br /> Provider to County with respect to remounting two existing Orange County Ambulances <br /> onto Ford Type-1 chassis: <br /> a) Orange County Remount #22-5-2 2310 to include refurbishment as outlined in <br /> Attachment "A", #2252 2310 Orange County Remount Quote — Remount Pricing <br /> Version 18.3, dated 1-8-19. <br /> Amend Attachment#A, Remount Pricing Version 18.3 as follows: <br /> Amend"Original Excellance Unit Number to #2310 instead of#2252. <br /> Except for the change made Paragraph l.a.i)a) and to Attachment A herein, the Original Agreement shall <br /> remain in full force and effect to the extent it is not inconsistent with this Amendment.
